Both forms are required. If your physician is not filling out the Physical Fitness Certification, you will bring completed forms to the health office for confirmation that physical was done within 12 months of the application.  

Papers are submitted to Sheila Falciano in the main office once completed. She will finish processing them and contact the student/parents accordingly.
